Norman Regional Health System Community Health Needs Survey

Norman Regional Health System is currently conducting a community health needs survey for the regional communities it serves. The results of the survey will provide guidance to address identified health care needs and in the development of wellness and prevention programs.

Information will be gathered using both a paper and web-based survey.

By taking ten minutes to complete this 36 question anonymous survey, community members have an opportunity to state which health needs have the most impact on their quality of life.

Completed surveys that are not mailed in can be dropped off at the following two locations:

  • Norman Regional Hospital
    901 N. Porter Ave., Norman
    THE SOUTH ENTRY
  • Norman Regional HealthPlex
    3300 HealthPlex Parkway, Norman
    THE MAIN ENTRY

Conducting this assessment reflects Norman Regional’s long-standing commitment to identify the most urgent health needs and better target resources that improve the health of the communities we serve. NRHS will report the findings from the assessment on their website, NormanRegional.com, once the data is compiled.
