Skilled Nursing Facility Preferred Providers

To help patients transition to a different care facility, Norman Regional has created a post-acute care network.

Many patients go home after their hospital stay, but a patient may need a few days at a skilled nursing facility before returning home. Most patients stay in a skilled nursing facility (SNF) for about ten to 14 days. While you are there you receive nursing care and physical therapy to help you get stronger.

There are a lot of SNFs to choice from, and the decision of which facility is right for you can be difficult. That is why your hospital case manager and physician are available as a resource to help you select a SNF that is right for you.

Although there are lots of skilled nursing facilities to choose from, there are only a select number of facilities chosen to be in the Norman Regional Health System post-acute network. We strongly recommend you consider one of these post-acute care facilities when making your selection.

SNFs in the post-acute network work closely with Norman Regional to improve how your care is provided during your stay. The post-acute network facilities also work to reduce your length of stay so that you can get home sooner and they work with you to prevent an unnecessary readmission to the hospital. In fact, these facilities have to meet specific requirement to be a part of our network.
