Managing multiple prescriptions or chronic conditions? Our Medication Therapy Management (MTM) Clinic connects you with a clinical pharmacist for a personalized review of all your medications. This service helps ensure safe, effective treatment and prevents medication errors.
Medication Therapy Management (MTM) is a service in which a patient meets
one-on-one with a clinical pharmacist to undergo a comprehensive review
of the patient’s entire medication profile.
The pharmacist helps to optimize therapeutic outcomes, identify and prevent medication errors, and provide patient counseling.
Norman Regional Health System offers this clinic free of charge to help patients take their medications safely and effectively.
Patients who could benefit the most from an MTM Clinic visit are those with several medication prescriptions, multiple chronic disease states or any number of specialty medications.
If medication-related problems are identified, the clinical pharmacist will reach out to your referring provider to make recommendations regarding changes in medication therapy.
To be seen in the MTM Clinic, patients must be referred by a physician
for an appointment. Speak with your provider about being referred to this
clinic if you are interested in scheduling a visit.
The Norman Regional Health System Pharmacy is here to help you manage your medication therapy through the MTM Clinic by providing review and guidance on your prescriptions.
